Our Team

Members of our hospice team make regular visits to assess the patient and provide compassionate care and a member of our team is available to patients and families whenever the need arises. We develop an individualized plan of care that meets each patient’s need for pain management and symptom control related to the terminal illness.

Hospice Case Managers

Dunes Hospice Registered Nurse Case Managers provide hands on nursing care to patients. Hospice Case Managers also coordinate specialized care for medical, social and spiritual care.

Spiritual Counselors and Social Workers

Dunes Hospice social workers are experts in providing emotional support. Hospice social workers are able to provide suggestions about the patients health and help make funeral arrangements. Dunes Hospice chaplains are specialized in providing nondenominational spiritual care to patients and their families.

Hospice Aides

Dunes Hospice aides provide dignified compassionate and meaningful care to our patients under the supervision of a Registered Nurse (RN) Case Manager.

Administration

Dunes Hospice administration are liaisons in the Northwest Region of Indiana reaching out to help anyone suffering with a terminal illness to ensure they are not alone in the beautiful process of End of Life.
